California Community Transitions (CCT)
The CCT program is a Medi-Cal funded program available through the State Department of Health Care Services (DHCS). As a CCT Lead Organization, Virtue Home Care gives people the extra help they need to move from a medical facility back into the community.
Q: What is the process for enrolling into the CCT program?
A: Talk to a Virtue Home Care transition coordinator in your area. The Coordinator will come and meet with you to develop a plan. You will talk about moving to the community of your choosing. This could be Assisted Living, home, independent living, or Section 8.
Q: How does the CCT program help me?
A: Thei program will help identify the many services available in the community including Case Management, In-Home Supportive Services (IHSS), Nursing Care, Medical Equipment, and transportation.

Eligibility for CCT
- Medi-Cal covered 21+
- Currently living in a Skilled Nursing facility or other institutionalized care setting.
- Interested in moving back into the community.
- Able to reside safely in the community with appropriate support services.
